Cyan's shoelace is the only physical clue to Terry's mysterious recovery from cancer. Terry's nightmares mirror Al's experiences. Spawn enters Hell's fifth level where he is revered as a returning savior. He is forced into a confrontation with the Savage Dragon and defeats the dragon for the position of the Messiah.
